Who Qualifies for This $50 Copay?
Eligibility for the $50 monthly copay under this program is contingent upon having Medicare Part D coverage (either through a standalone prescription drug plan or a Medicare Advantage plan) and meeting specific clinical criteria based on Body Mass Index (BMI). Beneficiaries with a BMI of 35 or higher automatically qualify. Those with a BMI between 30 and 34.9 can qualify if they also have a co-occurring condition such as heart failure, uncontrolled high blood pressure, or chronic kidney disease (Stage 3a or above). Furthermore, individuals with a BMI between 27 and 29.9 may be eligible if they have a history of a heart attack, stroke, prediabetes, or symptomatic peripheral artery disease. It's important to note that if a patient already has a GLP-1 medication covered by their standard Medicare Part D plan for conditions like Type 2 diabetes, severe sleep apnea, or fatty liver disease, they should continue to obtain it through their existing plan.
Important Considerations and Accessing the Program

As a separate bridge program, the $50 monthly copay does not contribute to your annual Part D deductible or your yearly out-of-pocket spending cap. Additionally, beneficiaries cannot combine this program with drug manufacturer coupons or Extra Help (Low-Income Subsidy) discounts to reduce the $50 price further. To access these medications, eligible beneficiaries cannot simply pick them up from the pharmacy. A doctor must submit a formal prior authorization request confirming that the patient meets the necessary BMI and health criteria. If a patient is already successfully on one of these drugs, their doctor can attest to their BMI at the time therapy was initially started.
